Personal Health Record systems
. . . intend to empower individuals to manage their own health care. A personal, private and portable personal health records will enable individuals to maintain comprehensive, up-to-date histories for themselves and their families. Records also will be portable, enabling individuals to continue using the records even if they change employers, health plans or doctors. Google is working to be in to it and here is an example just announced this week.

The Discussion and Debate section of Healthcare Policy-Politiques de Santé offers a forum for essays and commentaries that address:
1) important health policy or health system management issues; or
2) critical issues in health services and policy research.
Submissions should be a maximum of 2,000 words exclusive of (no more than 20) references. The main points of the paper should be highlighted in an abstract (summary) of 100 words or less. For more information or to submit a piece please see the Author Guidelines.

In 2004, the Canadian Adverse Events Study (Baker et al. 2004) determined the incidence rate of adverse events (AE) in Canada to be 7.5%. This translates to approximately 185,000 for the almost 2.5 million annual hospital admissions in Canada. The study noted "close to 70,000 of these AEs were potentially preventable."
In March 2005, a "Good Catch" program was implemented in Edmonton's Capital Health Region, one of the largest integrated health regions in Canada, as part of the region's comprehensive system of reporting, analyzing and managing incidents, adverse events and near misses.
